Your counselor wants to make the most of your visit at CBC McKinney. For your first visit you may either arrive 15 minutes early and fill out intake forms in our reception area, or download them from this site and bring them with you already filled out. Your personal information is protected by state law. On your first visit, your counselor will review with you that and other rights and responsibilities as a client of CBC McKinney.

Suggestion: Open the following forms, print them on your printer, fill them out, and bring them to your first appointment. The forms require Acrobat Reader. In the unlikely case that you don't have Acrobat Reader, you can download a free copy from Adobe.
